Terms & Conditions – Online Query Submission

These Terms & Conditions will govern the use of our online inquiry / query form by our website visitors as well as visitors referred by our Social Media pages.

• By submitting the online query form, you are hereby granting permission to DPlus Diagnocare’s representative to contact you and arrange for a sample collection from your residence / place of work on a date and time of your convenience.
• Our team would be happy to assist you through the process and will make complete arrangements, right from collection of the sample to providing you with the test reports.
• Should you have a doctor’s prescription for the tests, please do upload the same. Please note that the prescription will be treated as confidential.
• On receiving your online query, our team will be in touch with you to reconfirm the same and a trained & experienced Phlebotomist will then visit you as per the indicated time from the given sample collection address. He /she will also collect other samples if required according to your prescription, subject to your providing spot consent for the same.
• The sample will then be transported to our lab in specially designed container in order to ensure that the integrity of the sample is maintained. Thereafter, our highly experienced lab technicians will be processing the sample.
• Upon completion of the test, our in-house Pathologists will verify / validate the results and a report will be generated. This report will be shared with you through electronic means (Email / WhatApp etc / Website Log-in). The time taken for the report will depend on the parameters being tested.
• Please be advised that by sharing your email id / phone number you agree that we may send you text no notifications, marketing offers and follow-up health check-up reminders. You hereby provide your consent to receive communication from DPlus Diagnocare and this consent will override any DND instructions that may otherwise be in place.
